4Ds Learning Overview
Learn how to identify and record "operational friction" using the 4D's framework: Dumb, Dangerous, Difficult, and Different. This guide covers the complete workflow from logging pre- or post-task insights to the supervisor learning review and resolution process.
Purpose of the 4D's Module
The 4D's Operational Learning module is designed to capture the reality of work as it happens. Its primary goal is to identify "operational friction"—the specific things that make a job harder, riskier, or less efficient than it needs to be.
Unlike standard hazard reporting, this tool encourages workers to pause and reflect on four specific dimensions ("The 4D's") to identify systemic issues. This module now includes AI capabilities to help transcribe discussions and auto-fill report details.
The "4D's" Explained
When completing a record, you will be asked to comment on the following four dimensions if they apply to your current task:
- DUMB: Is there anything that feels like a waste of time or doesn't make sense? (e.g., confusing rules, redundant paperwork, steps that add no value).
- DANGEROUS: Is there any step where you are relying on luck or it just feels risky? (e.g., near misses, high energy sources, relying on skill rather than controls).
- DIFFICULT: Is there anything physically awkward, hard to reach, or mentally taxing? (e.g., poor lighting, heavy lifting, jury-rigging tools).
- DIFFERENT: Is anything different today regarding crew, tools, weather, or parts? (e.g., new supplier parts, change in sequence, relief operator).
Creating a 4D Learning Record
- Navigate to the 4D's module and select New Record.
- Hierarchy: Ensure the correct Company, Division, and Site are selected.
AI-Assisted Auto-fill (Optional)
You can use the AI tool to speed up the reporting process:
- 4D Conversation: Type a freeform description of the situation or paste a transcript of a recorded discussion about the task.
- Auto-fill Button: Click Auto-fill. The system will attempt to populate the Contextual Information and 4D Assessment fields based on your description. Note: This will overwrite existing data.
Contextual Information
If you did not use Auto-fill, or need to make adjustments, complete the following:
- Observer/Team: Identify who is observing or performing the work.
- Date: The date of the observation.
- Task & Role: Describe What task is being performed and the Role of persons performing the work.
- Specific Work Area: Describe the specific location.
- Asset/Equipment: Link a specific asset if the friction is related to equipment.
- Reviewer/Accountable: Select the person responsible for reviewing this insight.
The 4D Assessment
Fill in the text areas for Dumb, Different, Difficult, and Dangerous. You do not need to fill in every section, only those that apply to the current situation.
Risk & Improvements
- Risk Rating: If something Dangerous was noted, provide a risk estimation.
- Regular Occurrence: Indicate if this is a regular occurrence (Yes/No).
- What worked well?: Capture positive feedback on controls or practices that made the job easier or safer.
- Suggested Improvements: Propose how the task or process could be changed.
- AI Suggestion: You can click the Suggest button to have the AI propose improvements based on the data entered in the 4D fields.
- Additional Context: Add any extra information that helps make sense of the observations.
Submission
- Submit for Learning Review: Moves the record to the next stage and notifies the Reviewer.
The Review Process (For Supervisors/Reviewers)
Once a record is submitted, the assigned Reviewer performs the "Learning Review".
- System Type: Categorize what part of the system caused the friction to help identify upstream trends:
- Design Issue
- Process-Procedure
- Tooling-Resource
- Environment
- Training
- Supply/Parts
- Other
- Actionable Status: Indicate if the observation is actionable at this time (Yes/No).
- Follow-up Type: If actionable, select how the insight was handled:
- Quick Fix: The issue was resolved immediately.
- Action Required: A formal Action is needed (link below).
- Hazard Created: The insight revealed a significant risk (link below).
- Details: Provide details regarding the review or the decision made.
- Workflow Actions:
- Close 4D Learning: Finalizes the record.
- Return to Draft: Sends the record back to the author for revision.
- Reject and Archive: Marks the record as rejected/archived.
Linking Records
This module integrates with other system areas. In the Associated Actions/Hazards section, you can:
- Actions: Log a new Action or link an existing one.
- Hazards: Link a Hazard record if the 4D assessment identified a significant safety risk.
