Creating and Submitting a Permit to Work Request

Step-by-step guide on how to create a new Permit to Work, fill in the required details, and submit it for authorisation.

Creating a New Permit

  1. Navigate to the Permit to Work module.
  2. Select 'New Record' (or a similar button like 'Create Permit') to open a new permit form. It will start in a 'Draft' status.

Completing the Permit Form

Ensure all mandatory fields (often marked with an asterisk *) are completed.

Key Details

  • Permit Applicant, Permit Holder Affiliation, Permit Holder: Identify who is requesting the permit and who is responsible for the work.
  • Type of Permit: This is a critical selection. Choose the specific type of high-risk work. This choice will dynamically display relevant safety checklists and questions later in the form.
  • Other Workers Involved: If 'Yes', you'll be prompted to list them.
  • Isolation Required: If 'Yes', specify the 'Type of Isolation'.
  • Work Area and Activity: Provide a clear and detailed description of the location and the work to be performed.
  • Authoriser: Select the designated individual or role responsible for approving this type of permit. This is essential for the permit to proceed through the workflow.

Permit-Specific Safety Sections

Based on the 'Type of Permit' you selected, one or more sections with specific safety questions and checklists will appear. These are tailored to the risks associated with that work type (e.g., Confined Space Entry, Excavation). Carefully review and complete all items in these sections.

Associated Records & Attachments

  • Associated Actions/Permits: Link this permit to any relevant corrective actions or other permits. You can often create new linked records directly from here.
  • Attachments: Upload supporting documents like Safe Work Method Statements (SWMS), Risk Assessments, site plans, or equipment inspection records.

Saving and Submitting for Authorisation

  • Save (Draft): Click 'Save' to save your progress. The permit remains in 'Draft' status, visible primarily to you, allowing for further edits.
  • Create Permit / Submit for Authorisation:
    • Once initial details are entered, a 'Create Permit' (or similar) button might move the permit to an 'Open' status, indicating it's a valid application but not yet ready for approval.
    • When all sections (including permit-specific checklists and Authoriser selection) are complete, click 'Submit for Authorisation' (or a similar button). This formally sends the permit to the designated Authoriser and changes its status to 'Requested'. Notifications are typically triggered at this point.
    • The 'Authorise' option for the approver may depend on the completeness of these permit-specific checklists.
