Take 5 - Stay Alive: Step-by-Step Assessment
This is the core of the Take 5 process, guiding you through critical safety checks with a series of questions.
Step 1: Stop - Understand the Task
Consider your preparedness and equipment:
Do I have the skills, knowledge, training to do this job?
Do I have a valid license, induction, permits to conduct this task?
Do I understand the steps required to do the task?
Do I have the right tools to do the job?
Are the tools in good condition and fit for purpose?
Do I have the right PPE & Safety Equipment? (e.g. Traffic cones, signage)
Have ALL isolations been completed?
Have I controlled manual handling risks?
Is the area barricaded to protect people from dropped objects?
Am I clear of moving parts? (Line of Fire)
Important: Answering "No" to any of these may mean you need to stop and address the issue before proceeding.
Step 2: Look for the Hazards
Identify potential dangers:
After controls are in place - can the hazards of this job cause me (or someone else) serious harm?
Will I be conducting high risk tasks (i.e. hot works, working at heights, confined spaces)?
Important: Answering "Yes" here signals a higher risk situation needing careful management or escalation.
Step 3: Assess the Risk
Evaluate the overall risk:
What is the level of risk?
(Select from: Low, High)
Step 4: Control the Hazards
Confirm your safety measures:
Have I identified and controlled ALL the hazards?
Does it feel right?
Important: If you answer "No", do not proceed until hazards are controlled and you feel it's safe.
Step 5: Do the Job Safely
If all previous steps are satisfactory, you can proceed.
Logging Detailed Hazards and Controls
If you need to document specific hazards and their controls in more detail, use the Identified Hazards and Controls link or button available in this section.
- This will open a separate area or form where you can:
- Describe the Hazard.
- Detail the Controls implemented or planned.
- Saving this information links it back to your main Take 5 assessment. This is useful for keeping a comprehensive record of how specific risks were managed.
