Linking Related Records and Supervisor Review

After completing the main assessment, the Take 5 form allows for connecting it to other relevant safety records and includes a section for formal review.

Associated Records

You can link your Take 5 to other items in the system:

  • Associated Actions: If your assessment identifies the need for corrective or preventative actions, you can link them here or log a new Action directly.
  • Associated Hazards: If specific hazards are identified that need to be tracked in the Hazard Management system, link them or log a new Hazard.

Review Section

If your Take 5 indicates high risks or unresolved issues, it will typically be sent for supervisor review.

  • Review Comments: Supervisors use this area to provide feedback or document their review of your Take 5 assessment. This creates a history of the review process. These comments are usually required if the Take 5 enters a 'Review' stage.
