How do I log a new incident in the system?
To log a new incident, navigate to the Incident Reporting module and select the option to create a new report. It is important to provide accurate initial information, as this will guide the subsequent investigation and processing steps. 1
Steps for the Initial Report:
- Navigate to the Incident Reporting module and select the option to create a new incident. 1
- Enter Basic Details:
- Verify the Hierarchy (e.g., Site, Department) is accurate.
- Enter the Date Reported, Date Occurred, and Time Occurred. The 'Date Occurred' must not be a future date and is vital for analysis.
- Provide a specific Location.
- Write a clear, concise Brief Description, as this is often used in notifications and lists. 1
- Make Critical Classifications:
- Classification (Outcomes): Select all outcomes that apply (e.g., Injury, Environment, Near Miss). This is a key step as it determines which detailed information sections will appear later for the Supervisor to complete. 1 2
- Confidentiality: Choose 'Standard' for most incidents. Select 'Legal Privilege / Confidential' for sensitive incidents, which restricts access and routes the report to designated Confidential Monitors. 1
- Describe the Event:
- Provide a full narrative of what happened in the Details section.
- Describe any Immediate/Temporary Controls that were put in place at the scene. 1
- Assign Responsibility:
- For 'Standard' incidents, assign the appropriate Supervisor.
- For 'Confidential' incidents, assign the relevant Confidential Monitors. 1
- Add Attachments: Upload any relevant photos, documents, or other evidence. 1
- Submit the Report:
- Save: Saves the report in a 'Draft' state if you need to complete it later.
- Submit: Formally submits the report, moving it to 'New' status and notifying the assigned Supervisor or Confidential Monitors. 1
After Submission
Once submitted, the incident moves to a 'New Report' status, and the assigned Supervisor or Confidential Monitor takes over to process it. This involves verifying details, assessing actual and potential risk, and completing any outcome-specific sections based on the classifications you selected. 3
