How Supervisors or Confidential Monitors should classify, assess risk, and complete initial details for new incident reports.

Once an incident is in 'New Report' status, the assigned Supervisor or Confidential Monitor takes over.

Initial Review and Processing

Verifying and Adding Specifics

  • Review the initial report details.
  • Complete fields regarding the task being performed, whether it was part of normal duties, and details of any other loss or damage.
  • Clarify potential harm to personnel or the environment if not already classified as such.
  • Record details if the incident was reported to the police.

Defining Risk: Actual and Potential

This is a critical step influencing the investigation's depth.

  • Actual Incident Category: Using the Risk Matrix, assess the actual consequence and likelihood of the event that occurred.
  • Potential/Worst Case Incident Category: Again using the Risk Matrix, assess the potential severity if controls had failed or were absent. A 'Significant Incident' rating here often mandates a more thorough investigation (like an ICAM) and formal sign-off.
  • Externally Reportable?: Indicate if the incident requires reporting to external bodies and provide details.

Completing Outcome-Specific Sections

  • Based on the 'Classification (Outcomes)' chosen by the reporter, fill in all relevant detailed sections. For guidance on these, see the "Completing Incident Classification Outcomes Details" document.

Linking Initial Actions and Hazards

  • Associated Actions: Log any immediate corrective actions taken or needed.
  • Associated Hazards: Link any hazards that contributed to the incident or were newly identified. Refer to "Actions Management" or "Hazard Management" documentation for more on these processes.

Next Steps

  • Investigate: If a formal investigation is required (often determined by the Potential Risk), this action moves the incident to 'Open' status.
  • Close Off: For minor incidents not requiring extensive investigation or formal sign-off (based on risk), a direct 'Close Off' option might be available.
  • Save: To save progress and return later.
