Logging a New Incident: Initial Report
Step-by-step guide to creating and submitting an initial incident report.
Starting Your Report
Navigate to the Incident Reporting module and select the option to create a new incident.
Essential Initial Information
Hierarchy and Basic Details
- Your organizational hierarchy (e.g., Site, Department) may pre-fill. Verify its accuracy.
- Date Reported, Date Occurred, Time Occurred: Ensure the 'Date Occurred' accurately reflects the event date, not just the reporting date, as this is vital for analysis. The 'Date Occurred' cannot be a future date.
- Location: Be as specific as possible.
- Brief Description: Provide a clear, concise summary. This is often used in lists and notifications.
Critical Classifications
- Classification (Outcomes): Select all outcomes that apply (e.g., Injury, Environment). This choice determines which detailed sections will appear later for the Supervisor to complete, so accuracy here is key.
- Confidentiality:
- Standard: For most incidents.
- Legal Privilege / Confidential: Choose these if the incident is sensitive. This will restrict access and may change who (Confidential Monitors) handles the report.
Describing the Event
- Details: Provide a full narrative of what happened.
- Immediate/Temporary Controls: Describe actions taken immediately at the scene to ensure safety or mitigate further impact.
Assigning Responsibility
- Supervisor: For 'Standard' incidents, assign the appropriate Supervisor.
- Confidential Monitors: For 'Legal Privilege' or 'Confidential' incidents, assign the relevant monitor(s).
Attachments
- Upload relevant photos, documents, or other evidence.
Submitting Your Report
- Save: Saves the report as a 'Draft' if you need to complete it later.
- Submit: Formally submits the report, moving it to 'New' status and notifying the assigned Supervisor or Confidential Monitors.
