Adding Questions
How Administrators can add question fields with predefined answers, scoring, comments, and linked records to a form.
The Question tool allows adding structured questions where users select from predefined answers (e.g., Yes/No/N/A, Safe/Unsafe/N/A, Custom). Features include mandatory settings, field help, scoring, colour coding, conditional comments, attachments, and linked records.
Steps to Add a Question Field

Open the form in the Form Editor.

Drag the 'Question' button to the desired location.

Enter the heading for the question set in the 'Name' field.

Optionally, 'Add field help' to guide users.

Set 'Mandatory' to Yes or No.

Under 'Questions', select 'Add', enter the question text, and select 'Ok'. Repeat for all questions in this set.

Select the 'Answer type' (e.g., Yes/Safe (Green) No/Unsafe (Red) N/A (Grey), or Custom to define your own buttons, colours, and scores).

Choose whether to 'Display Scores' (Yes/No).

For each question, check the boxes to enable 'Comments', 'Attachments (media)', and/or 'Linked records' as needed.

Configure 'Advanced validation rules' if needed (e.g., require a comment if 'No' is selected). See 'Form Editor: Advanced Validation Rules'.

Configure 'Linked Records' if enabled. See 'Form Editor: Linked Records'.

Select 'Ok' to add the question set.

Select 'Save' at the bottom of the Form Definition page.
