Completing Incident Classification Outcomes Details

Reference guide for the specific information required based on the 'Classification (Outcomes)' selected for an incident.

The 'Classification (Outcomes)' selected (e.g., Injury, Environment, Vehicle) determines which specific information sections you need to complete. This is typically handled by the Supervisor or Confidential Monitor.

Injury

This section connects to the Injury Management module. You will typically log a new, linked Injury Report, detailing the person involved, injury type, and circumstances. Full management of the linked Injury Report often involves specific Injury Manager roles.

Environment

Provide details such as:

  • Type of environmental incident (e.g., Spill, Emission).
  • Substance spilled or lost, and what caused it.
  • Area and species impacted, including estimates of area size or number of species.
  • Nature of impact (e.g., Contamination, Erosion), contaminant type, volumes released/recovered, and duration.

Equipment

Document details of any equipment involved:

  • Type, make, model, and identification numbers (fleet, registration).
  • Insurance information if applicable.

Vehicle

This links to a dedicated Vehicle Details form. Log a new Vehicle Details record (or link an existing one) to capture make, model, registration, and insurance. Separately, describe any damage to the vehicle directly within the main incident form.

Violence and Abuse

Record:

  • Names or descriptions of person(s) involved.
  • Availability of CCTV or telephone recordings.
  • Nature of the incident (e.g., Verbal Abuse, Physical Assault).

Community Complaint

Capture:

  • Complainant's name and contact details.
  • Nature and full details of the complaint (e.g., Noise, Dust).

Security

  • Indicate if a security area was breached.

Psychosocial

  • Specify the nature of psychosocial factors (e.g., organisational like workplace culture, or environmental like noise).

Production, Near Miss, Other

While these may not have extensive unique sections, ensure all standard incident details are thoroughly completed.

  • Production: May include fields for "Apparatus involved."
  • Near Miss: May include fields like "What happened unexpectedly?" or "What was the worker doing?".
