Initiating an ICAM
The ICAM form is used for comprehensive root cause analysis of significant incidents. It's typically launched from a main Incident Report by selecting 'ICAM' as the "Root Cause Investigation Type" and then choosing to log a new ICAM record.

The ICAM form itself progresses through 'Draft', 'Open' (active investigation), and 'Closed' (completed) statuses.
Key Sections of the ICAM Form
Plan
- Document the date the ICAM planning/investigation started and confirm if an investigation plan document has been uploaded.
Analyse
This section uses structured questions to identify absent or failed defenses across various categories. For each identified deficiency, you can describe it and link to Action records for remediation.
- Categories include: Absent or Failed Defenses (e.g., PPE, Warnings), Task/Environment Conditions (Human Factors like competency, and Workplace Factors like equipment integrity), Organisational Factors (e.g., Communication, Procedures), and Individual/Team Actions (e.g., Non-compliance). Refer to "Actions Management" documentation for creating and managing these actions.
Investigate
- Determine What Happened: Provide a detailed account of the sequence of events established during the ICAM.
Fix
- Fixes: Describe the corrective actions and solutions derived from the ICAM. Ensure specific actions are also formally logged.
Report
- Approval: Designate an approver for the ICAM report if required by your process.
Completing and Linking the ICAM
- Fill out all relevant ICAM form sections thoroughly.
- Attach the full ICAM report document and any supporting evidence directly to the ICAM record.
- Progress the ICAM form to its 'Closed' status (e.g., via a 'Complete' button). Once completed, the ICAM record remains linked to the main Incident Report, providing crucial input for the incident's overall resolution and sign-off.
