Signing Off an Incident (Authoriser Guide)
How Internal Authorisers review and finalize completed incident investigations.
When an investigation is submitted for sign-off ('Pending Sign Off' status), the designated Internal Authoriser performs the final review.
Authoriser's Review and Sign Off Process

Access and Review: Open the incident. Thoroughly review all aspects:
- Initial report details, classifications, and risk assessments.
- All outcome-specific information.
- The complete investigation report, including contributing factors, root causes, and recommendations.
- Any linked records like Actions, Hazards, and especially the full ICAM report if one was conducted. Ensure these are adequate and appropriately actioned.
- All attachments.
- Comments from the Supervisor/Monitor.

Decision and Comments:
- Navigate to the "Sign Off" section.
- Comments: Add your own comments regarding the review and decision.

Workflow Actions:
- Sign Off: If the investigation is satisfactory and actions are complete or well-planned, select this to move the incident to 'Closed'.
- Re-open: If further work is needed, select this to return the incident to the Supervisor/Monitor (typically to 'Open' status). Clearly state reasons in your comments.
- Reassign Authoriser: If you are not the correct person, an option to reassign may be available.
- Save: To save comments and return later if more review time is needed.
